Critical Illness Insurance (CI) in Malaysia: Frequently Asked Questions

1. What is critical illness insurance?

Critical Illness (CI) insurance pays a lump sum if the insured person is diagnosed with a covered critical illness and meets the policy’s claim definitions and requirements.

For example, if you have RM100,000, RM200,000, RM300,000, RM500,000 or RM1 million in CI coverage, the applicable insured amount may be paid to you when a qualifying claim is approved, subject to the policy’s payout terms.

Unlike a Medical Card, which generally pays eligible hospital and treatment expenses, CI insurance provides cash that you can use as you choose. You may use it to replace income, pay your mortgage, support your family, cover recovery expenses or give yourself time to rest without worrying as much about finances.

A Medical Card helps with medical bills. CI insurance helps protect your financial life.

2. Why is critical illness insurance important?

If you are diagnosed with cancer, have a heart attack or experience a stroke, your Medical Card may help with hospital bills. But what happens if you cannot work for a year or two?

Your mortgage, car instalments, children’s education and household expenses continue even if your income stops. A CI payout can help replace income, so you can focus on treatment and recovery while maintaining your family’s lifestyle.

As a financial-planning guideline, we often suggest considering about five years of annual income for CI protection. The amount should be adjusted for your savings, family responsibilities and financial commitments; it is not a universal requirement. For example, five years of income at RM5,000 a month is RM300,000.

The aim is not simply to get through a serious illness. It is to have room to recover without putting unnecessary financial pressure on your family.

3. How much critical illness coverage should I buy: RM100,000, RM200,000, RM300,000, RM500,000 or RM1 million?

The right amount depends on your income, financial commitments, dependants, savings and what you can afford.

Here is an income-replacement illustration:

Monthly income 5 years of income replacement
RM3,000 RM180,000
RM5,000 RM300,000
RM8,000 RM480,000
RM10,000 RM600,000
RM15,000 RM900,000
RM20,000 RM1,200,000

These are planning illustrations, not required coverage amounts. Whether you choose RM100,000, RM200,000, RM300,000, RM500,000 or RM1 million, the question is whether the amount can help protect your financial responsibilities during a difficult period.

An eligible CI lump-sum payout is paid to you; it is not limited to reimbursing medical expenses or supported by hospital receipts. The actual payment depends on the plan, covered illness stage, policy definitions and applicable benefit limits.

4. Should I choose 36 critical illnesses or more than 150 critical illness conditions?

A longer list of covered illnesses does not automatically make a plan the better choice. The number is one part of the decision.

The adviser-provided comparison for this article lists these Allianz Malaysia options and figures. Counts and covered-condition definitions depend on the product generation and version; refer to the latest official policy contract for the cover you are considering:

  • HealthCover: 36 critical illnesses.
  • HealthCover Plus: 70 critical illnesses, with additional protection for certain childhood illnesses.
  • Prime Care+: more than 150 critical illness conditions across early, intermediate and advanced stages.

Faith’s rough estimate for planning purposes is that a basic 36-illness plan might cost around half the premium of a more comprehensive plan. This is not a market quotation, an expected discount or a guaranteed price relationship. Actual premiums and any meaningful comparison depend on age, health, benefit structure and coverage amounts, and require individual quotations. The plans do not necessarily provide identical cover; this estimate does not mean the same cover is available at half the price.

As a coverage-amount illustration, not a same-price quotation: would you prefer RM500,000 of protection against 36 major critical illnesses, or RM250,000 under a plan covering a wider range of conditions?

There is no single right answer. The more comprehensive plan may offer valuable early-stage benefits, while the higher sum assured may provide more financial protection against major illnesses. The choice depends on your needs, budget and the risks you want to protect against.

The right CI policy is not necessarily the one with the most covered illnesses. It is the one whose protection amount, features and cost suit your circumstances.

5. How do I select the right critical illness insurance policy?

Start with three questions:

  • How much would my family need if I could not work for several years?
  • What can I comfortably afford to pay for long-term protection?
  • Do I want protection mainly for major critical illnesses, or do I also value early-stage and broader coverage?

Then review the covered illnesses and their definitions, waiting periods, survival requirements, payout percentages, exclusions, and whether a CI claim affects other benefits.

Do not assume that all benefits are payable at the same time or can simply be added together. A payout depends on the illness stage, benefit limits, claim approval and any applicable waiting or survival requirements. Depending on the policy, a CI payment may reduce another benefit. Separate riders are not necessarily additive, and multiple claims are payable only where the policy terms allow. Check the specific contract rather than assuming these features apply to any particular product.

A sound plan balances the protection you want with what you can reasonably maintain over time. Suitable cover that stays affordable may be more useful than an expensive plan that becomes difficult to keep.

6. Can I purchase critical illness insurance in Malaysia if I have a pre-existing medical condition?

You may still apply. Acceptance depends on the nature and severity of the condition and the insurer’s underwriting assessment.

Possible outcomes include standard acceptance, additional premiums, specific exclusions, postponement or rejection. Disclose your medical history accurately when applying.

7. Is critical illness insurance in Malaysia tax-deductible?

Qualifying CI premiums may be eligible for personal income tax relief in Malaysia, depending on the policy structure and the prevailing Inland Revenue Board of Malaysia (LHDN) requirements.

Eligibility and deductible amounts depend on the applicable tax rules and how the coverage is classified. Check the latest LHDN guidelines or consult a qualified tax adviser.

8. How do I make a critical illness insurance claim?

After a diagnosis, contact your insurance agent or insurer as soon as possible.

You will generally need to submit a claim form, medical reports, investigation results and any other supporting documents the insurer requests. The insurer will assess whether the diagnosed condition meets the policy’s CI definition and claim requirements.

If approved, the eligible benefit will be paid according to the policy’s terms and conditions.

9. Is there a survival period for critical illness claims?

Some CI policies require the insured person to survive for a specified period after a qualifying diagnosis or event. The period varies by product and policy terms.

Waiting periods, exclusions and illness definitions may also affect eligibility. Check your policy contract for the exact requirements.

10. How can I apply for critical illness insurance with Allianz Malaysia?

Choosing CI insurance is not simply a matter of picking the plan with the longest list of covered illnesses. It is about protecting your income and your family’s financial stability, and having room to recover without unnecessary financial stress.

Options range from 36 major critical illnesses to more extensive multi-stage coverage. A financial needs analysis can help you weigh protection against affordability.
